On September 10, 2018 (Monday), you are directed to be present during the Flag Raising Ceremony at the Port of Manila Grounds for your Oath Taking Ceremony. Be there in any White Business Casual Attire no later than 7:00a.m.

After the Oath Taking Ceremony please proceed to the Human Resource Management Division to receive notice of your appointment.

Be reminded that the following requirements must be submitted to the Human Resource Management Division prior to assumption of duties of your position, and on/before September, 28 2018 (Tuesday):

7.    Authenticated copy (from the issuing agency) of Eligibility/License with Original Official Receipt (1 original copy)

8.    Certificate of Assumption (4 original copies)

9.    Position Description Form (4 original copies)

10. I.D. Update/Renewal Form with 1 pc. 2×2 I.D. picture

11. For EG/IG appointees:  Neuro-Psychiatric Exam Result

12. Diploma/Certificate of Graduation (1 – certified copy by the school/university)

13. Transcript of Records

(1 – certified copy by the school/university)

14. Certificate of Live Birth (1 authenticated copy/original copy issued by the PSA)

15. Statement of Assets, Liabilities and Net Worth as of Assumption Date (4 original copies)

16. BIR 1902 or 2305 Form (1 original copy)

17. UMID Card (1 photocopy) if applicable

18. GSIS Membership Form (1 original) if applicable

19. Pag-Ibig Membership Form (1 original copy) if applicable

20. Philhealth Membership Form (1 original copy) if applicable

“If the appointee has taken his/her oath office and assumed the duties of the position, he/she shall be entitled to receive his/her salary at once without awaiting the approval/validation of his/her appointment by the Commission.”

Pursuant to Section 20, Rule VI of the Civil Service Commission (CSC) Memorandum Circular No. 24, s. 2017 (2017 Omnibus Rules on Appointments and Other Human Resource Actions), “An appointment issued by the appointing officer/authority may be cancelled if the appointee does not assume office or report to duty within thirty (30) calendar days from receipt of the written notice of appointment.”

Further, please be guided by Section 17, Rule VI of the same Memorandum Circular which provides:

“Sec.17. An appointment issued in accordance with pertinent laws and rules shall take effect immediately on the date it was signed by the appointing officer/authority. The date of signing shall be indicated below the signature of appointing authority.”
