NOTICE TO ALL APPOINTEES TO THE POSITIONS OF SVCOO, COO V, ITO I AND CMT II

April 13, 2018

CONGRATULATIONS!

We wish to inform you of the issuance of your promotional/original appointment dated April 12 and 13, 2018 to the following positions:

Computer Maintenance Technologist II (CMT II);
Information Technology Officer I (ITO I);
Customs Operations Officer V (COO V); and
Supervising Customs Operations Officer (SVCOO).

On April 16, 2018, please coordinate with the offices indicated below to receive a copy of your appointment:

The Human Resource Management Division for appointees under :
a. Groups
b. Manila International Container
c. Ninoy Aquino International Airport
d. Port of Clark
The respective Administrative Officer for the appointees under other collection districts.

Pursuant to Section 20, Rule VI of the Civil Service Commission (CSC) Memorandum Circular No. 24, s. 2017 (2017 Omnibus Rules on Appointments and Other Human Resource Actions), “An appointment issued by the appointing officer/authority may be cancelled if the appointee does not assume office or report to duty within thirty (30) calendar days from receipt of the written notice of appointment.”

Further, please be guided by Section 17, Rule VI of the same Memorandum Circular which provides:

“Sec.17. An appointment issued in accordance with pertinent laws and rules shall take effect immediately on the date it was signed by the appointing officer/authority. The date of signing shall be indicated below the signature of appointing authority.”

In this regard, you are hereby requested to comply with the following documentary requirements and submit the same to the Human Resource and Management Division (HRMD) within three (3) work days upon receipt of your appointment:

Submission of one (1) original copy of the Certificate of Eligibility/Rating/License Authenticated by the Issuing Agency and the corresponding Official Receipt (i.e. CSC, Professional Regulation Commission, and Supreme Court);
Re-submission of three (3) original copies of Duly Accomplished Personal Data Sheet (PDS) with Work Experience Sheet (CS Form 212 Revised 2017) submitted last 03 April 2017 (Include all designations and positions held, and trainings attended as of 03 April 2017); and
Re-submission of a copy of the Accomplished Individual Performance and Commitment Review for the last two rating periods (January to June 2017 and July to December 2017).

Please note that only those appointments with complete supporting documents shall be forwarded to the CSC for attestation and approval.

An official Memorandum containing further instructions shall be sent to you and the head of your Group/Collection District on April 16, 2018.
